Synagis and who gives first dose

We at The Medical University of South Carolina are looking at possibly changing our practice and were curious what other units were doing.  I look forward to hearing from everyone. 

 We would like to know if other units give Synagis as inpatient prior to discharge or as outpatient within Synagis season?  If they give as outpatient who administers it? An independent home health agency, hospital based HH, a certain clinic, etc.

Hello :)
We have the APA guidelines hanging in our unit....and if the baby meets criteria, they get the shot by the NICU RN before they are discharged. The patient will than follo up in our high risk clinic in 28 days for the next shot.
The guidelines are pretty straight forward, often times we need to remind the physician to order the shot before the baby goes home.

Yes, we have the same process in place. Here at VBMC Brownsville Texas, we give Synagis during RSV season (November throu April), as inpatienet the first dose is given before they are discharge home during RSV season and the second dose is follow up as outpateitn by Pediatrician. The paperwork sometime can be a problem, so we send the information to the Pedi office at time of discharge so they can have the parents complete it before the Synagis is due
The criteria we use is documented (admitting diagnosis) 35 weeks gestation on admission

Here at UNC in Chapel Hill we give Synagis to inpatient infants in the NICU. On the 15th of every month, beginning November 15th, all infants that qualify for Synagis in the NCCC will receive a dose throughout the entire season. The pediatrian will continue the monthly dosing as an outpatient once the infant is discharged. We usually call the pediatrian to update them, that way if there is paper work that is needed, it can be started early. If a patient is being discharged a day or two before the 15th, we'll go ahead and dose early that way it doesn't get missed.
